To submit a link account request the learner must have the following information:
- First name - fuzzy match for MLTSD Account would be accepted, for example would accept Tim or Timothy for Timothy)
- Last name - exact match would be expected
- MLTSD Learner ID
- Postal Code (optional)
- Year of Birth (optional)
The learner should be submitting this form as there are questions regarding consent which are required
